If you would like to sign up for any of the Study Tour programs, please fill in these forms, print out and mail them to:

Be sure to sign these forms where indicated, and please include required deposit for your tour (see Tour Information) to secure your place. Please make all checks payable to ARTIS. If you are unable to print out these forms or you wish to speak with us please call 1-800-232-6893.

We have prepared these forms in a manner that allows you to type your information prior to printing. Or you may print the forms and fill them in by hand before returning to ARTIS.
